The Chemung County Fifth Annual "Walk a Mile In My Shoes"
Suicide Prevention Walk will be held on:

Saturday, May 8th, 2010
11:00 am  to 1:00 pm
Eldridge Park, Elmira, NY

We invite you to be a part of this special event!

On line registration is now closed. Please join us for on the spot registration as described below:

Individuals can even register at our ON THE SPOT Registration booth the day of the walk.  The only difference is that T-shirts and Lunches will be available on a first come-first serve basis until they are gone.

Directions for Teams:

• FIRST, gather members for your team and register your team.  You can register using our Online Team Registration Form or you can print out and fax/mail us the Team Registration Form (PDF File).
• SECOND, print out the Team Captain Checklist & Instructions (PDF File). Read this through and make note of the dates for the different steps you'll be taking.
• THIRD, print out a copy of the Team Member Donation Form (PDF File) for every member of your team.  Encourage your team members to begin collecting donations.

Directions for Individuals:

• FIRST, register yourself using our Online Individual Registration Form or by printing out and faxing/mailing us the Individual Registration Form (PDF File).
  
• SECOND, print out the Individual Checklist & Instructions (PDF File). Read this through and make note of the dates for the different steps you'll be taking.
  
• THIRD, print out a copy of the Individual Donation Form (PDF File) and begin collecting your donations.

• Feel free to print out and share our "Save the Date" Promotional Flyer (PDF File) to let others know about our upcoming walk.
